Thank you for your interest in becoming a Senior Rep for Effervescent Media Works for the year of 2011

Below are outlined details regarding the program. Please read over these all carefully and fill in the information below.

As a Senior Portrait Representative you must:

Have great style and lots personality

Enjoy being in front of the camera

Be a Junior or Senior in High School

Attend a school located in Salt Lake County

Be available to have the session completed in the month of March or April in Salt Lake City

Agree to tell your friends and classmates about Effervescent Media Works and share your senior portraits with them

Have your parents or legal guardian sign a model release form and Agree to represent only Effervescent Media Works

Use these images as your senior pictures

And in Return you will receive:

A Senior Portrait Session

50 + Edited Images

A Cd with all you images in their highest resolution

The copy right release to all your images

An online gallery of images to share with friends

Oppurtunities to earn cash and free products

By signing on to be a Senior Portrait Rep for Effervescent Media Works you will be eligible to earn cash and free prints. The more referrals you generate the greater your rewards. Parental consent is required to participate and you must complete the below application. Once you have been chosen as a Senior Rep you will be photographed during the spring season of your junior or senior year. Within 2 weeks of the session you will receive your pictures on a CD for your to use on social networks and to print at your discretion(the copy right is included). What is a Senior Portrait Rep Model?

They are spokes models that help promote Effervescent Media Works with their own amazing portraits. You're going to tell your friends about how incredible your experience was anyway, so why not earn some cash while you're at it? An ideal senior rep model has lots of friends and is a very outgoing and energetic person. He or she may also be involved in extra curricular activities and has a winning personality. We need reps who are going to create a buzz about EMW and who are going to have lots of friends to tell about it!

What do Senior Portrait Rep Models Do?

I am not asking you to be a sales person….my photos are going to sell themselves. I am asking you to simply tell all of your friends about Effervescent Media Works and show off your pictures, create a buzz about my photography and your experience with me. It's simple really. The more friends you get to come get their senior portraits done with me, the more free stuff you'll get and money you'll make. We also have great incentives for the people you refer, so it's a win-win.

What do I receive for being a Senior Portrait Rep Model?

•You will receive a free Senior Portrait Session with unlimited clothing changes. You'll get to pick a location in Salt Lake City

• After the session you will receive 30 personalized referral cards to give to friends and classmates with your name, your portraits, and our business information to hand out to friends. This is what you'll hand out to all your friends to get them to come get their portraits done with us.

• These cards should be handed to out to fellow 2013-14 grads from any high school. Mom and Dad can help, too! Senior Reps will receive credit for each student that books and completes a session so it's important that the rep card with your name be presented at the time of booking. The more you show off your portraits and promote Effervescent Media Works the more credits you can earn.

• The Student that was referred and shows proof with the Senior Rep card will also receive 20 % off the usual listed total price. Then for every person that that person refers they will receive 1 sheet of portraits free. So something for everyone.

• You will also get several of my favorite photos as low resolution files to share on social networking sites like facebook, myspace or blogs with my logo on them. Along with an agreement that you will keep one of those photos as your profile picture while you are a senior. • Discounts on your print orders (applied until the end of Senior portrait season) • Referral

Rewards:
1-10 - $20 per booked referral
11-19 - $25 per booked referral
20+ - $30 per booked referral plus you will also receive a free 5x5 photo album.

When your referrals book a paid session and bring in your rep card you will receive the above mentioned amount according to the amount of referrals you bring to us. These referrals can include anyone who books the following sessions: Senior, Newborn, Family Portraits, Portraits, Fashion, Weddings, etc..

Do you have your parent's permission to apply as a spokes model? (If your parents have any questions or concerns, please have them call me at 801-583-1655 and I'd be happy to answer any and all questions they may have. They can also visit the website at http://effervescentmediaworks.com to see my work) Interested students should submit all their information by March 31st, 2011

If you are chosen you will have a creative Senior Portrait Session in March, April or May of 2011.
Reps will be chosen in early March and April by Effervescent Media Works- Photography and Design. Reps will be contacted if chosen and also announced on the blog as well as on Facebook. No more than 3 reps will be chosen from each school.
